Record of Death or Injury.

6th October 1896 7pm North Sea. A Smith,3rd hand. Whilst assisting to shoot the port gear and engaged in shackling on the fore bridles to the warp. The fore bollard gave way owing to the strain of the warp on it, due to the heavy sea and flying off struck the deceased on the head causing such serious injury that death ensued. Smith never regained consciousness.

Record of Casualty.

6/10/96 North Sea. Fore bollard on port rail & main rigging broken on same side of vessel. Due to the strain on the warp on account of the heavy sea which was running, which caused the bollard to break, & the warp aswell as the bollard to fly off.
