Record of Death or Injury.
7.30am 9/12/1906 J Cookson deck hand. The deck hand went forward to secure a pound board that was washing about the deck & whilst in the act the vessel fell in the hollow of a sea & gave a lurch to port. Cookson was lost sight of but mate thought he had jumped into a doorway, until he heard a cry. Engine was put astern but he sank as soon as we got alongside of him.

I certify that after enquiry I am satisfied the loss of the deck hand J Cookson was in accordance with the circumstances reported on page 15 & therefore that no further investigation is necessary.
Record of Casualty. 26th October 1906 5.45pm. Grounding, a blinding snow storm. She came off immeadiately & sustained no damage.
