Contains applications by individuals to join the Friendly Society, note that some applications are accompanied by a medical officer's certificate of examination of the individual making application, whilst some have no accompanying examination certificate, and in a number of cases only the examination certificate is present. Applications involved a 1 shilling 'proposition fee' and requested the following information to be provided [although please note that not all pieces of information are provided on each application]: Name in full; Address; Occupation; Age on last Birthday; If married and if so whether it was according to Jewish Rites; Wife's Name and age; Any children; Whether parents are alive; Any brothers or sisters; Whether applicant is member of another society; Whether applicant has ever drawn sick benefits from such a society; Whether applicant has ever been refused by any society and if so why; Whether applicant or wife suffer from any chronic or periodic complaint; Whether applicant has ever undergone surgical operation; Whether applicant is subject to cough, cold or bronchitis; Signature and date; Witness; Proposer and Seconder. Examination certificates provide name, address and occupation of person examined, state whether person is in a fit or unfit state of health, and whether they are eligible or ineligible to be admitted to the Friendly Society, and are dated and signed by examining Medical Officer.
