7/3/07 R Lewis spare hand. Injured his back by slipping & falling off the bridge on to the deck.

I certify that after enquiry. I am satisfied that the report on page 15 hereof of the circumstances under which A J Knudsen lost his life, is in my opinion correct & true and that further enquiry is unnecessary.
